Tattoos may be erased in many methods before lasers, including salt abrasion, dermabrasion, removal of acid and open surgical excision. They were quite efficient, although they were unpleasant and left scars. Originally, laser removal began as a means of burning off a tattoo. Lasers like carbon dioxide and argon lasers have been tried but they were not much superior than destructive procedures and left open wounds which had to heal over time. A breakthrough event of laser tattoo removal was the introduction of Q-switched lasers. These lasers operate using a mechanism entirely distinct from ablative lasers (the kind that destroy the skin). A q-switch is an extremely high-speed shutter which may emit a very brief laser light explosion in the nanosecond range. The light, all packed into one brief explosion, is likewise pushed out to a very high energy. The impact is like putting an ice cube into a glass of warm water – it breaks. The heat of the laser is preferentially absorbed by the tattoo pigment, but it happens more rapidly than the particle expands thermally, resulting in smaller fragments fracturing. These tiny parts become small enough for the immunological white cells of the body to be digested and taken to remote regions. As this is done, until it is almost undetectable, the tattoo is lighter and lighter.

Various ink hues react to the laser therapy differently. The market is now divided into 3 Q-switched lasers: Ruby, Alexandrite and Nd:YAG. The ruby was the first q-shift laser with a wavelength of 690 nm. This makes it more effective than others, like black or red, for blue and green tattoos. At 755 nm, Alexandrite is best suited to green tattoos. The YAG may be used with 532 or 1064 nanometers such that it works more effectively than other lasers against both black and red tattoos. Some hues seem to be more laser resistant than others following laser therapy. Yellow, pink, and purple are among them.

Every procedure for laser tattoo removal is quick. A 3-inch tattoo may take up to half an hour or more yet only 30 seconds can take a single laser treatment! Topical anaesthesia is often applied before treatment or local anaesthesia may inject a limited region. After therapy, blistering and bruising occur sometimes, although this is not required for the process. Treatment typically takes place every four to six weeks. After 3 sessions, a single tattoo may be almost gone, although this is not common. Tattoos take 6-12 therapies more often. Some inks, because of their chemical composition, are resistant to laser tattoo removal. This applies to tattoo pigments made of metal oxide such as iron and titanium oxides. While still detachable, numerous further treatments are needed.
